UPDATE 5/1/2024

Change Healthcare (a division of UnitedHealth Group), the billing and claims processing agency for Lakeshore Radiology, is down due to a cyber attack that took place on 2/21/2024. ​Systems began to be restored on 4/29/2024, though we still anticipate that it will take 1-2 more weeks to be fully operational.

As such, information regarding billing and payments from Lakeshore Radiology is not available.  If you have questions about a bill and/or are concerned about a late payment, please know that because of this outage, a grace period will be applied to any current bills if needed.

​Lakeshore Radiology billing is performed via a third party, Change Healthcare based in Michigan.  If you have any questions regarding your bill, please contact Change Healthcare at (866) 744-1456 and have your account number available.  Please be aware that the Lakeshore Radiology Associates charges are those incurred for the professional, physican service.  

Lakeshore Radiology does offer the opportunity to pay your bill online at https://www.peryourhealth.com/​

Please Note:  You may receive a separate statement from the facility where your examination was performed for technical fees.  This is usually a separate bill from Froedtert Holy Family.  If you have questions regarding your bill from Froedtert Holy Family, please contact Froedtert Holy Family billing via their website, via phone at (920) 320-2591, or via email at pbs@hfmhealth.org
