Billing Information

​Lakeshore Radiology billing is performed via a third party, Change Healthcare based in Michigan.  If you have any questions regarding your bill, please contact Change Healthcare at (866) 744-1456 and have your account number available.  Please be aware that the Lakeshore Radiology Associates charges are those incurred for the professional, physician service.  

When services resume, Lakeshore Radiology will offer the opportunity to pay your bill online.​

Please Note:  You may receive a separate statement from the facility where your examination was performed for technical fees.  This is usually a separate bill from Froedtert Holy Family.  If you have questions regarding your bill from Froedtert Holy Family, please contact Froedtert Holy Family billing via their website, via phone at (920) 320-2591, or via email at pbs@hfmhealth.org
